Waiver

MINOR Movement Instructor LIABILITY WAIVER AND RELEASE FORM

I Do Hereby Give Consent for my child to participate in an Enrichment, Recreation, Martial Arts, and Movement Lesson  program with Moses McCarter. I recognize that the programs/classes require physical exertion that may be strenuous at times and may cause physical injury and I am fully aware of the risks and hazards involved. 

I understand that it is my responsibility to consult with a physician prior to and regarding my child's participation in all activities, programs/classes or use of equipment. I represent and warrant that my child has no medical condition that would prevent his/her participation in the programs/classes. 

I agree to assume full responsibility for any risks, injuries or damage known or unknown which my child may incur as a result of participating in the programs, classes or use of equipment. I agree that by my child participating in these physical exercise sessions or movement activities, is entirely at their own risk. This includes, without limitation, (a) My child’s use of all amenities and equipment in the facility and any off-site location and my child’s participation in any activity, class, program, personal training or instruction, (b) the sudden and unforeseen malfunctioning of any equipment (c) instruction, training, supervision, or dietary recommendations. I give consent for my child to voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury. 

I knowingly and voluntarily and expressly waive any claim I may have against the Instructor Moses Mccarter for injury or damages that my child may sustain as a result of participating in the programs, classes or by use of equipment. 

I forever release, waive, discharge, and covenant not to sue Moses Mccarter for any injury or death caused by their negligence or other acts. I understand that misuse of equipment may result in injury. 

I have read the above waiver and release of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above I acknowledge that I have carefully read this waiver and release and fully understand that it is a release of liability. I agree to voluntarily give up any right that I may otherwise have to bring a legal action against the personal trainer or instructor for negligence, or any other personal injury or property damage or loss action.
